When you have an upcoming visit planned, you now can download and print out our "Patient Registration Form" fill it out before your visit. That way we'll be able to see you much faster when you arrive.
Our "Patient Registration Form" is in .pdf format which is viewable with the "Acrobat Reader Plug-in". Most newer computers come with this plug-in already installed.
